Our supported scheme works to improve your patients confidence, creating sustainable change in their daily routine & improve their referred medical condition.

What is Exercise Referral?

If you are suffering with a pre-existing health condition or recovering from an illness or operation, you could benefit from Exercise Referral at Canterbury Christ Church University.

Our scheme can help improve overall health, increase fitness levels, and support long-term lifestyle changes with the guidance of our team of highly qualified Exercise Referral Practitioners. Giving you the knowledge and confidence you need to address your referred condition in a safe and effective way.

 

We work with patients who are “inactive” doing less than 30 minutes of moderate physical activity per week.

• Commited to making a long term lifestyle change.
• Aged 18 +  
• Have an existing stable health condition/ recovering from an operation.
• Clinically stable and compliant with their medication.

• Alcohol/Drug rehabilitation
• Joint/mobility/ musculoskeletal  
• Obesity
• Mental health/ emotional wellbeing
• Completion of cardiac rehabilitation 
• Hypertension
• Cardiovascular disease
• Diabetes
• Cancer

• GP
• Therapist
• Physiotherapist
• Occupational Health
• Health Care Assistants 
• Doctors and Nurses
• Dietician
• Practice District Nurse
• Hospital Consultant
• Respiratory or exercise physiologists

Simply complete an Exercise Referral Form and have it signed by your Exercise Referral Practitioner.

You will have confidence that you will receive high quality support from our specialist team.
