Occupational Health Questionnaire for Students on Professional Programmes
Teaching, Nursing, Midwifery, Occupational Therapy (OT), Radiography, Social Work and other professional occupations are all highly demanding careers involving close contact with people at vulnerable stages in their lives. It is necessary therefore that candidates for these professions should be physically and mentally fit to enter those particular health professions. It is recognised that people with disabilities or chronic illnesses may make a very valuable contribution to these professions. The purpose of the Occupational Health Questionnaire is to give the University Medical Advisers information about the candidates' health status to help them to decide if they are fit for a career in one of these professions, or if they are likely to have the need for special provision to assist in their training and/or subsequent employment.
Only candidates who have been offered a place at the University, and who intend to accept it, are required to complete the questionnaire.

Students who are required to attend Medway Maritime Hospital Occupational Health Service must first telephone for an appointment and take the appropriate Immunisations and Blood Test form with them. For further information please contact occupational.health@canterbury.ac.uk.
