This application form has been designed as an aid to guide employees and Heads of Department through the process of making and assessing an application for flexible working. It is not however mandatory and an employee may make a request in free form in a letter, fax or e-mail if they prefer. N.B. there are certain things that must be included in a written request, so check the guidelines for this.
These guidelines give detailed information for employees and Heads of Department on how to assess an application. Following these will ensure compliance with the procedure laid down by the law.

Please note the following if you are thinking of making an application:
- You should check the eligibility criteria in the Guidelines for Employees
- It could take up to 14 weeks to process a request so you should make your request in good time
- If a request is agreed, there will be a permanent change to your terms and conditions and there will be no automatic right to revert to your former working patterns
- Any flexible working pattern involving a reduction in hours will result in a reduction of pay
