If you are accessing this web page from outside campus, you will be required to provide a username and password. When prompted, use the same username and password as you would to logon to a University computer on campus, with the username in the following NEW format:
ccad\username (e.g.: ccad\abc12)

If the login prompt has three boxes (username, password and domain) then you need enter only your username (abc123) into the username box and enter cc into the domain box.
